AFFIRMATION AND IMPORTANT NOTICE
I affirm that all the information contained in my resume
and additional related documents provided with my application are true
and correct and that no material information has been omitted.
I affirm
that I have not mis represented any information.
I understand and agree
that if I submit any false or misleading information or omit any material
information, it may result in an offer of employment being withdrawn
or, if I am already on site, I may be dismissed.
I understand and agree that I will not be offered employment
until:

PS&S, Inc and any potential employer has determined that
I am qualified for employment

And that my prospective employment has been approved
by a potential employer.

That any offer of employment and any resulting employment
will be conditional upon:
- Satisfactory completion of medical exam as prescribed
by any potential employer and the visa section of the Saudi Embassy.

By obtaining an appropriate visa for entering the Kingdom
which
includes a satisfactory medical report, a police clearance
report and other information prescribed by the embassy.

PS&S, Inc. and any potential employer obtaining satisfactory
references and confirmation of qualification.
I further understand and agree that:

That PS&S may elect, at it's complete discretion, not
to submit my application for employment to any potential employer.
- That the decision of any potential employer as to the
suitability of my application for employment is made solely by the employer
and not within the control of PS&S, Inc.
- That I will not become an employee of PS&S, Inc..
PS&S, Inc. shall in no circumstances have any liability for any loss
or damage whether consequential or otherwise arising in connection with
any offer of employment and/or any resulting employment or any decision
not to offer employment.

I further understand and agree that if I should, by personal
choice, resign or terminate my contract during the first three months I
may be responsible for the cost of my return ticket to my home at the discretion
of the employer.
I further understand and agree that the first 90 days
of any employment contract are considered the "probationary period" and
the employer reserves the right to cancel the contract without cause during
this initial period at their own discretion. I also understand that this
action is not taken lightly as the employer will already have expended
sums on my behalf to commence my employment.
